Led by Sara Gena Israel, APRN, PMHNP-BC, the program incorporates behavior change science, lifestyle medicine principles, and personalized support to help individuals build realistic habits that fit their lifestyle and long-term wellness goals.

The focus is not on perfection or rigid rules, but on providing supportive guidance, practical tools, education, and accountability in a way that feels realistic and sustainable for everyday life.

Rx in Balanced Mind Weight ManagementRx™ refers to an evidence-informed lifestyle prescription that includes education, skills, and supportive accountability. It does not refer to medication prescribing, medical diagnosis, or treatment.

Step 1: Lifestyle & Mental Wellness Intake

Every client begins with a structured 60-minute intake designed to build a personalized foundation for change. This intake is self-pay and not billed to insurance.

Areas reviewed may include:

  • Nutrition awareness and eating patterns
  • Sleep and daily routine review
  • Stress and coping patterns
  • Mood and emotional well-being check-in (non-diagnostic)
  • Weight history and health context review
  • Readiness for change
  • Collaborative goal setting

If medical evaluation or labs may be helpful, clients are encouraged to discuss this with their clinician or request a separate appointment through Balanced Mind Mental Health.

After your intake, you and Sara will determine which support option best aligns with your goals, preferences, and clinical fit.

Pathway access is determined individually based on clinical fit, goals, and current availability. Some women benefit from coaching alone, some from the therapy pathway alone, and some from both. During your intake, we will determine what fits where you are.
